Below are links and descriptions to a number of forms that are commonly used in the Athletic Department.

• Physical Examination Form 24-25

  This form is completed and submitted as part of the Athletic Clearance Process, which is done online under the Registration tab.

• Driver Registration Form

  Please fill out this form if you are a parent volunteering to transport other students and/or a parent giving permission for their student to drive themselves. Copies should be submitted to the head coach or the athletic office.

• CIF-SS Parent/Student Transfer Form

  All students transferring to South High School after the start of 9th grade, regardless of transfer type, will fill out this worksheet and submit it to the Athletic Director. A transfer flow chart can be found here and the Athletic Director will work with you on the appropriate transfer type and requirements.

• Independent Study Forms 2024-2025

  All students applying for Independent Study PE credit, including those for off-season athletics classes that are not offered on campus (ie off season Tennis, Girls Soccer), must fill out this packet in its entirety and submit it to the main office.
